I’m Sam Johnson, a board-certified psychiatric nurse practitioner dedicated to providing compassionate, personalized care. I specialize in medication management while recognizing that true healing involves more than just medication. Together, we’ll explore your goals, strengths, and challenges to create a treatment plan that supports your mental, emotional, and physical well-being.

PRISM is more than just our name, it is both a powerful metaphor and a meaningful acronym that reflects our values: Personal Growth, Resilience, Inspiration, Self-love, and Mindfulness.
We provide evidence-based psychiatric care rooted in compassion and collaboration. Just as a prism brings clarity and transformation by breaking light into its full spectrum, we aim to help you better understand your mental health, manage symptoms, improve daily functioning, and support the full expression of who you are.
The acronym PRISM reflects the essence of what we aim to achieve together. Each letter stands for an important aspect of the healing process:

What is medication management?
Medication management is a collaborative process where we work together to determine if medication can support your mental health goals. It includes:
Evaluation and Assessment: We take the time to understand your mental health history, current symptoms, and goals.
Personalized Treatment Planning: If medication is appropriate, we’ll discuss your options and collaborate on a plan that feels right for you.
Ongoing Monitoring and Support: Your progress matters. We’ll meet regularly to assess how your treatment is working and make adjustments as needed.
Education and Empowerment: We’ll provide clear, compassionate information so you can make informed decisions about your care.

Meet Your Provider
Hi, I’m Sam Johnson, a board-certified family and psychiatric nurse practitioner and the founder of PRISM Mental Health. I’ve been practicing as a family nurse practitioner since 2016 and have specialized in psychiatry since 2020. My background in both physical and mental health allows me to take a comprehensive, whole-person approach to care.
My style is collaborative, compassionate, and rooted in education. I take time to understand your history, your goals, and what matters most to you. From there, I offer personalized recommendations and information so that you can make informed decisions about your care. I want you to feel like an empowered participant in your mental health journey, with support that extends beyond just medication.
At PRISM, I strive to create a welcoming, inclusive, affirming, and safe space for everyone. A place where you can be your true self without fear of judgment. I believe that healing begins when we feel seen, valued, and accepted for who we are.
When I’m not working, I’m probably at a concert, up in the air practicing aerial arts, or planning Halloween decorations way too early. I love live music, comedy shows, movies, and any excuse to travel somewhere new, especially if it involves laughter or something a little weird and wonderful.

We provide medication management for a range of mental health conditions. Our sessions may also include brief psychotherapy, guidance on coping strategies, and recommendations for additional therapies or lifestyle adjustments that can support your well-being.

We provide care for individuals age 15 to 64 who are experiencing a range of mental health concerns. We treat a range of conditions such as: depression, bipolar disorder or other mood disorders, anxiety disorders, ADHD, PTSD and trauma-related symptoms, OCD, and more.

Your first appointment will typically last 50 minutes. We will discuss your mental health history, current symptoms, and goals for treatment. Together we will create a personalized care plan that may include medication management, therapy recommendations, or other supportive treatments.

Click the Book Now button on our website and you’ll be taken to my Headway scheduling page. From there, you can choose an available time, check your insurance, and book your appointment online.

Yes! We offer secure telehealth appointments for your convenience.

We ask that you provide at least 48 business hours notice if you need to cancel or reschedule an appointment. Late cancellations or missed appointments may incur a fee of $100

We do accept some insurance through Headway. When you click Book Now you will be directed to my Headway page where you can find out more about insurances. Through Headway you can check your insurance benefits. If we are out-of-network we can provide a superbill.

Booking your first appointment
Your first appointment at PRISM Mental Health is scheduled through Headway, our trusted partner for managing everything related to payments and insurance to make things as easy as possible for you.
Clicking “Book Now” will take you to our Headway booking page, where you can view current availability and select a time that works for you. From there, you’ll be prompted to create a Headway account, enter your insurance or payment information, and complete any necessary forms.
You’ll be able to see your cost per session ahead of time and check your insurance benefits through the Headway portal. If you ever need help with billing, insurance, or your account, you can contact the Headway team directly for support.
